I'm not sure if the random number generator gets streaky when save/reloading, or if there's hidden modifiers at difficulties above the easiest, but some of the streaks I've seen defy logic and probability. On a 50/50 sniper shot, I've missed it a dozen times in a row and given up. On something that's 10% crit fail, I've save/reloaded and had crit fail 6 times in a row, no non-crit fails or successes in between.
